- April 07 – April 10; 8a – 4:30p (site is aware these dates may not get filled due to compliance timeframes)
- April 14 – April 17; 8a – 4:30p (site is aware these dates may not get filled due to compliance timeframes)
- June 09; 8a – 4:30p
- August 28; 8a – 4:30p
- December 01 – December 04; 8a – 4:30p
- December 08 – December 11; 8a – 4:30p
- Clinic M-F 8a-4:30p
- Setting: clinic
- FTE: 1
- Required Procedures: Provider must be able to do SRS and SBRT.
- EMR: EPIC, Mosaiq
- Support Staff: One APP will provide coverage
- Reason for Coverage: vacation coverage
- Deal breakers (do not present this): Provider must be able to do SRS and SBRT.

- Treatment software: Eclipse treatment planning, Mosaiq record and verify, EPIC Electronic Health record
- Is there a Dosimetrist or Physicist ? Yes, both full time in the department.
SUBMISSION REQUIREMENTS :
- Board Certified - REQUIRED
- NE license - Highly Preferred
- SRS and SBRT - Highly Preferred
- Case logs required at time of presentation - client will require before interviewing/offering provider
- Please disclose any disciplinary actions taken on medical license (past or present) - REQUIRED
