SUBMISSION REQUIREMENTS

- Rates and specific availability MUST be submitted at time of name clear - will not be sent over without - Required, no exceptions
- Board Certified - Required
- ACLS - Required
- Active MI license & CSR at time of submission - Required no exceptions
- Clean background with no actions (minimum malpractice is OK) - Required
- All candidates should have prior experience (completed Fellowship with minimum of 2-3 years of experience)- Required no exceptions
- COVID Vaccine - Required
