Family Nurse Practitioner Locum Job in Arcata, CA
Schedule :
- 18 PPD, minimum to see 18 PPD
- Monday-Friday 8-5
Job Description :
- Candidate CVs need to have the month & date on their work experience w/all gaps in employment accounted for.
- Includes some Chronic Pain Management
- Chart closure within 72 hours
- Mix of peds/adults
- A decision is typically made within 7-14 days
- APC's with 3 years of experience in a similar setting
- PAs must have a CA Controlled Substance Education Course Certificate
- NPs must have a Furnishing License with Schedules II
- Active DEA needed before the start
- Credentialing takes 30-60 days (six weeks is standard for Locums who submit app./documents timely)
- The following immunizations are required for credentialing (before the start date):
- 2022-2023 Flu Vaccine,
- TB Status/X-Ray Results (results must be within 90 days before the start date),
- Positive MMR Titer or Complete Immunization Series,
- Positive Hep B Titer or Complete Immunization Series,
- Positive Varicella Zoster Titer or Complete Immunization Series,
- Tdap Immunization (must be within the last 10 years),
- HCO & Vendor agree to a 30-day notice for ending all assignments.
