Family Practice Locum Opportunity in Arizona

Specialty: Primary Care NP

Start Date: As soon as possible

Shift Details: 

• Five 8’s

Schedule: 

• Monday – Friday

• 7:00am – 5:00pm

Work Details:

• Support Staff: 4 MDs, 2 PAs

• Amount of Coverage Needed per Month: full-time, M-F

• Patients per day: 14-16 in 8 hours or 18-22 in 10 hours

• Patient Population/Age: 63 years old, however this clinic sees all ages

• Department Description: outpatient clinic

• EMR: Cerner

• Hospital Bed size: 99 beds

• Special Procedures:  none

• Experience Required: At least 5 years Primary Care clinic experience (to not include Jails/Prisons)

Practice Setting: 

• Clinic

Requirements:

• Board Certification: FNP

• License Required: AZ

• DEA: yes

• CSR:  no

• Certifications Preferred:  

• Certifications Required:  BLS

• Minimum Experience Required: 5 years as a FNP

Credentialing Timeframe: 30 days
