- Practice Setting: inpatient/outpatient
- Reason for Coverage: time off coverage
- Patient Population for Required Coverage: Adult, Geriatric
- What Percentage of the Practice is Hematology %, Oncology %, Internal Medicine %: 50/50
- Number of Physicians in Practice: 3
- How Many APPs Provide Coverage: 2
- Average Number of Patients Per Day - HOSPITAL: TBD
- Clinical Area: Oncology - Inpatient Setting, Oncology - Outpatient Setting
- Trauma Level/Designation: Youngstown level 3, Joes level 1, boardman non trauma
- Documentation System/EMR: EPIC
- Required Procedures: Chemotherapy - Intrathecal, Chemotherapy - Intracavitary - Intrapleural / Peritoneal, etc.
- Required Duties: Prescribes, Orders Images, Orders Tests
- Administration of Chemotherapy: Nurse oncologist
SUBMISSION REQUIREMENTS :
- Board Certified - REQUIRED
- OH License - REQUIRED
- ACLS Certified - REQUIRED
- MUST BE WILLING TO COVER LOCATIONS AS LISTED IN JOB
- AVAILABILITY MUST BE NOTED AT TIME OF NAME CLEAR - Preference to providers that can cover ALL dates
- Flu vaccine - REQUIRED at time of credentialing
- COVID Vaccine - Preferred
- DEA - Verbal Confirmation Required at Name Clear
