• Average Number of Patients Per Day - OUTPATIENT (Clinic/Office) :Rad Onc Treatments per day =18
• Average Number of Patients Per Day - INPATIENT  (Call/Rounding) : 0
• Trauma Level/Designation (Hospital): n/a
• Patient Population for Required Coverage: Adult
• Documentation System/EMR: Aria
• Will you Accept Providers Needing to Obtain State License?  Yes 
• Board Requirements (Board Certified/Board Eligible):  BC preferred
• Certification Requirement (BLS, ACLS, etc.):  BLS
• Required Procedures: Conventional Radiotherapy, 3D Radiotherapy, SBRT, External Beam Radiation - 3D-CRT, External Beam Radiation - IGRT, External Beam Radiation - IMRT, Stereotactic Body Radiation Therapy-SBRT
• Required Duties: Orders Tests, Prescribes, Orders Images
• Estimated Credentialing Timeframe:  60-90 days
