Sat/Sun 9AM-5PM

Setting: Walk In/Urgent Care with Primary Care available
Duties: 72 hours of Face to Face coverage per pay period as sole provider
Days per month: 14-16
Required Procedures: Routine
EMR: Allscripts/Altera/AEHR
Support Staff: Clinic shifts are 1 provider, 1 nurse, 1 front office

Board Certified - REQUIRED
Active MN License - REQUIRED
Residency Requirements/Fellowship - REQUIRED
Active BLS, DEA - REQUIRED
Clean Malpractice/Background – REQUIRED
Must state availability at name clear (# of shifts per month) - REQUIRED
Disclose if provider has previously worked at any other CHI/Dignity - REQUIRED
COVID VACCINATED - REQUIRED
