• 24/7 call coverage as needed
• Average number of cases per day: 1-2
• Patient Demographics: Adult and Geriatric
• Level 2 trauma
• Annual ER volume: 61,645
• Number of exam rooms: 1 bi-plane / 1 single plane
• 5 IR Physicians in practice
• Support Staff: RNs and Radiology Techs
• Supplies available: Stryker neuro coils, and both Stryker and Medtronic Flow-diverters. We have Abbott and Cordis Carotid stents
• EMR: Epic
• Procedures: Carotid Stent, Endovascular Coiling, Endovascular aneurysm repair(EVAR), etc.
• Additional procedural information/requirements listed in attached practice description
• Estimated Credentialing Timeframe: 60 days once cred application is submitted
