2 CRNA for 7P-7:30A

1 CRNA 24 hour weekend coverage

11 providers for OR 3 for Cath Lab

Submission Requirements

• MI License: REQUIRED, no exceptions (not accepting IMLC)
• NBCRNA certification - Required
• BLS, AANA and ACLS - Required
• Providers must have Clean Background/No Malpractice for quick credentialing - Required
