CREDENTIALING • License: Active only • DEA Required: Yes • Boards: Board Certified Only • Skills/Certs: Active CA license and DEA FLURO required (will need to be active or temporary fluro certificate) Board Certified or Board Eligible within 3-5 years of residency. Please consider starting to ensure your provider has all of these items to ensure they can qualify for privileges at the time of confirmation. o All HBS specialties require 2 years of current Hospital clinical activity in the specialty. o Gaps greater than 3 months must be explained. o Include current number of case logs upon presentation for surgical specialties.

ADDITIONAL DETAILS We are currently looking for one provider to cover 24hr call shifts at Santa Rosa starting in January. Shifts will be 730am-730am. Below are the specific shifts of coverage we need as of right now. More dates will be needed later in the year. Please note this is beeper call so the provider is not required onsite.
